Housing after mental health treatment: a planning checklist
A treatment transition involves both a care plan and a place to live. Use this guide to organize the housing arrangements and questions you want resolved with the people involved. Clinical discharge decisions belong with your treatment team.
Start with the place you will actually live
A referral, a promising conversation and a confirmed housing arrangement are different stages. Ask your current team what living arrangements should be considered, then check each provider's own admission process, support and availability.
- Returning to a family or existing home: discuss the resident's wishes, the actual space available and which everyday tasks each person agrees to help with.
- Considering a supportive residence: confirm fit, room arrangements, support boundaries, written costs and move-in approval with that residence.
- Exploring an independent rental: consider the housing agreement and the practical support that would need to be arranged separately.
- Needing public or subsidized housing help: ask the relevant agency about its application or referral path. Our Broward housing guide separates those starting points.

Five housing arrangements to confirm
- Destination and acceptance. Obtain the actual move-in confirmation, correct arrival address and contact person. Confirm how the timing fits with the treatment team's plan.
- Arrival. Agree who arranges transportation, when someone can receive the resident and what to do if arrival is delayed.
- Costs and supplies. Separate the amount due for housing from travel, personal supplies and outside services. Verify what the residence supplies before buying or packing.
- Outside appointments. Ask the treatment team to clarify follow-up arrangements and who handles any questions about medications, equipment or care instructions. Separately confirm the trip and any assistance needed to attend.
- Responsibilities. Ask who handles housing questions, clinical questions and practical move tasks. Confirm what information the resident wants shared and how necessary records should be transferred.

Keep a simple housing handoff record
Use a private paper or document. For each item, write the action, the person who agreed to do it and whether it is confirmed or still pending. Keep sensitive care details out of a general housing inquiry.
- Housing confirmation
- Provider, room arrangement, agreed move-in date and contact for changes.
- Arrival plan
- Who arranges the journey, who receives the resident and the delay contact.
- Payments and essentials
- Written amounts/due dates, belongings to bring and outstanding questions.
- Follow-up logistics
- Who confirms appointment details and the outbound/return transportation.
- Unresolved item
- What remains unanswered, who will check and when to follow up.
Resolve gaps before the day of the move
If housing approval, a room or transportation is still pending, tell the current treatment/discharge team. Ask what alternatives and timing changes should be considered. Do not treat a submitted housing inquiry as a reserved room.
For example, a proposed move on Friday may depend on a residence confirming arrival arrangements and someone confirming the ride. Mark both as pending until the responsible people agree. That is a logistics example, not an Ocean Breeze admission timeline.
