The inquiry form collects your first and last name, email address, message, and, if you choose, your phone number and whether you are contacting us as a resident, family member, caregiver, or referring professional.
How inquiries are used
Your information is used to respond to your inquiry, discuss housing fit and availability, and coordinate next steps. Inquiries are sent to the Ocean Breeze Living team by email. Professional referrals may also be sent to the referral contact.
Please keep medical details out of the form
This form is for general housing inquiries. Please do not include medical records, diagnoses, treatment histories, insurance details, or other sensitive medical information. Email and web forms cannot guarantee absolute confidentiality.
Website operation
The hosting and email providers process technical information needed to operate the website and deliver inquiries. This can include IP addresses, browser information, and request logs. The form uses a short-lived security token and temporary request limits to help prevent abuse.
Call tracking
We use CallRail to understand which website visits lead to phone calls. CallRail may replace the phone number shown on this site with a tracking number that forwards to our team. Its script collects visit and referral information and uses cookies to keep the same tracking number visible during your visit and when you return. Our website inquiry form is excluded from CallRail form tracking.
